It’s an interesting idea, but I suggest a role switch.? The doula is an unsympathetic character; the loss of her own child does not justify stealing someone else’s and killing the mother. It’s a morally repugnant act.
However, if you refocus the story, make the pregnant woman the protagonist, the victim of the desperate doula, then the premise could make for some compelling drama.
Another suggestion:? make the pregnant woman a newly arrived, illegal Latina, fleeing violence and poverty in her own country with no family or friends in the USA. Which makes her a natural target for the doula’s scheme:? who will know or miss her?

I second what dpg suggests. Flipping the roles of the prot- and the antagonist creates enoughempathy to follow the chilling story. Making the pregnant woman an illegal immigrant puts her in even more jeopardy and pressure (even before the psychokiller arrives). Plus, it can create an extra level if social commentary.
