      Why did he goto Sicily?? What drove him there?? Was he already seeing someone?? Was it at the advice of his therapist or doctor?? There are so many what if’s and why’s here.? Who did he fall in love with?? What brought them together?? Don’t be afraid to be clear.

      I agree with dpg, you should add some info about the problems that the protagonist will go through in his pursuit of happiness/normal life. What about the parents of these two children? You should add the boy’s nationality as well.
