Wait. If they're both comatose, how can they do anything at all, let alone falling in love again?
I think it's better if you change it to having amnesia instead, as another poster suggested.

the original concept was only one had amnesia and they didn't realise they wetter in a dream state

How about this:

"After a horrific accident two comatose lovers meet in a dream state with no memory and must fall in love again if they're to awaken."

Is there an Antagonist or obstacle besides their amnesia?
